Why Health Systems Are Starting to Benchmark Lab Quality, Not Just Certify It

A certification tells you a laboratory is legal to operate. It doesn’t tell you whether the people working inside it can do their jobs safely, efficiently, or without burning out. Those are two different questions — and health system leadership is starting to notice that only one of them has actually been getting measured.

For years, “the lab passed inspection” has stood in for “the lab is good.” It’s an understandable substitution — accreditation is the only number most facility leaders have had access to. But a passing grade on a compliance checklist and a well-functioning lab are not the same claim. Treating them as interchangeable is starting to cost health systems money, talent, and credibility with their own boards.

What compliance actually confirms

Laboratory accreditation exists to answer one question: does this facility meet the minimum standard required to operate safely and legally. CLIA sets the federal floor for any lab performing patient testing; CAP accreditation and ISO 15189 build on top of that floor with more detailed, laboratory-specific checklists (CAP Laboratory Accreditation Program) — but they remain checklists: pass/fail assessments of whether specific practices, controls, and documentation are in place. ISO 15189 does address the physical environment, requiring controls for layout, ventilation, and contamination (SimplerQMS, ISO 15189 overview) — but as a compliance requirement, not a graded measure of how well that environment performs day to day.

That work is valuable and non-negotiable. It’s also, by design, binary. A lab either has the required contamination controls, documentation, and quality-control processes in place, or it doesn’t. Certification was never built to tell you whether a layout supports clean specimen flow under real daily volume, whether a safety feature holds up under actual staffing pressure, or whether the people running the lab can sustain the pace without burning out. Those are questions of degree, not threshold — and they need a different kind of measurement.

What BioScore measures that a checklist doesn’t

Lab BioScore is built to answer the second question: not “does this lab meet the standard,” but “how well does this lab actually perform for the people and workflows inside it.” It looks at three areas a certification checklist isn’t designed to capture:

Workflow. How specimens, staff, and materials actually move through the space — whether the layout supports the way the work really happens, or whether people are routing around it every day.

Safety design. Compliance requires certain safety features to exist. It doesn’t evaluate whether those features hold up under the way the lab actually operates — under real staffing levels, real traffic patterns, real pressure.

Staff experience. This is the dimension a pass/fail checklist has no mechanism to capture at all, and it’s increasingly hard to ignore. Lab staff burnout is now a well-documented problem tied directly to safety and quality outcomes (Siemens Healthineers / Harris Poll survey, via CLP Magazine).

This isn’t a new idea in healthcare facility design broadly. The Center for Health Design and federal research bodies like AHRQ have built a substantial, peer-reviewed evidence base — well over 1,000 studies — showing that the physical design of a healthcare facility measurably affects patient and staff safety, satisfaction, and retention. Lab BioScore applies that same evidence-based lens specifically to laboratory environments, and turns it into a single, trackable number.

Why boards are starting to ask for a number

Health system capital committees are under more pressure than ever to show that facility spend produces a return — anticipated ROI is now the top factor in capital purchasing decisions for a growing majority of health system executives (HFMA, Health System Capital Investment Strategy 2026). Leading systems are moving from defending spend after the fact to building multi-year capital-performance dashboards and project-level ROI reviews (Mortenson, Hospital Capital Planning: 2026 Strategy).

A pass/fail accreditation doesn’t give a capital committee much to work with. It confirms the lab is legally allowed to operate — it says nothing about whether last year’s renovation improved workflow, whether this year’s request will reduce turnover, or how one facility compares to another in the same system.

Healthcare has already lived through this shift once. The Leapfrog Hospital Safety Grade emerged because pass/fail accreditation wasn’t giving patients, health plans, or hospital boards a way to compare facilities on actual safety performance. Leapfrog didn’t replace accreditation — it layered a graded score on top of it, because a single letter grade is easier to act on than a compliance file.

Lab quality is heading the same direction. A number a board can track quarter over quarter, compare across sites, and tie to a capital request is simply more useful than a binary pass — and that’s the gap Lab BioScore is built to fill.
